Prime Vitality is built around continuity and personalized attention. Whenever scheduling and clinical needs allow, patients work with the same provider, helping support a better understanding of their history, goals, response to treatment, and follow-up needs.

Do you treat children?
Age eligibility depends on the service requested, the patient’s needs, and the provider’s clinical judgment. Please contact the office before booking care for a minor so the practice can confirm whether the visit is appropriate.
What does “Prime Vitality” represent?
The name reflects the practice’s goal of helping patients protect their health, restore balance, and pursue lasting vitality through evidence-informed, personalized medical and wellness care.
What conditions may be evaluated?
Prime Vitality evaluates many common concerns that can be assessed safely by telehealth. Examples may include uncomplicated respiratory symptoms, allergies, selected urinary symptoms, minor skin concerns, cold sores, pink eye symptoms, medication-refill requests, and other non-emergency concerns.
What additional services are available?
Depending on medical history, examination needs, and clinical appropriateness, services may also include primary and preventive care, women’s health consultations, skin-condition care, hormone-replacement evaluation, personalized weight-management support, and travel-medication planning.
Can you prescribe medication?
When medically appropriate, prescriptions may be sent electronically to your preferred pharmacy. A prescription is never guaranteed and is based on the provider’s evaluation, your history, medication interactions, allergies, examination findings when available, and current standards of care.
Do you prescribe controlled substances?
Prime Vitality maintains a strict restricted controlled-substance prescribing policy and does not prescribe, refill, renew, replace, or manage any DEA Schedule I or Schedule II controlled substance, or any opioid medication classified under Schedule III — whether by telehealth or in-person. Do you offer hormone or weight-management treatment?
Prime Vitality offers evaluation and ongoing support for hormone health and weight management. Treatment may require laboratory testing, an in-person assessment, medical-record review, regular monitoring, and follow-up visits. Medication eligibility is determined individually; no specific medication or outcome is guaranteed.
Can you help with travel medicine?
Pre-travel consultations may address altitude-sickness prevention, malaria prevention, traveler’s diarrhea planning, and motion-sickness medication. Prime Vitality does not administer travel vaccines during a telehealth visit. Patients may be referred to a pharmacy, travel clinic, urgent care, emergency department, laboratory, imaging center, or specialist when needed.
What if telehealth is not appropriate?
If your symptoms require a hands-on examination, vital signs, testing, imaging, a procedure, or emergency evaluation, the provider will recommend the safest next step. Because in-person appointments are temporarily unavailable, concerns that need in-person care may be referred to another facility.
Can laboratory tests be ordered?
Laboratory testing may be ordered when clinically indicated and when ordering arrangements are available. Patients receive instructions about where to complete testing and how results and follow-up will be handled.
When should I seek emergency care?
Call 911 or go to the nearest emergency department for severe chest pain, severe trouble breathing, stroke symptoms, fainting, major injury, uncontrolled bleeding, severe allergic reaction, suicidal thoughts, or any rapidly worsening or life-threatening symptom.
How much does a telehealth visit cost?
A standard self-pay telehealth visit is $79 unless a different service, promotion, membership benefit, or fee is stated at booking. Specialized consultations, procedures, laboratory services, medications, and outside services may have separate charges.
Do you accept insurance?
Prime Vitality is currently self-pay and does not bill insurance. Payment is due at the time of your visit.
How does self-pay work?
Prime Vitality is a self-pay practice. You may request information about the expected visit fee before scheduling. The visit fee generally does not include pharmacy charges, laboratory testing, imaging, specialist services, or treatment costs unless specifically stated.
What payment methods are accepted?
Available payment methods are shown during scheduling or confirmed by the office. Payment may be required before or at the time of service. Please contact the office if you need help confirming an accepted payment method.
Are prescriptions and tests included in the visit price?
No. The visit fee covers the professional evaluation. Medication, laboratory, imaging, vaccination, pharmacy, and outside-facility costs are billed separately by the organization providing those products or services.
What is the cancellation or refund policy?
Please cancel or reschedule as early as possible. Fees for late cancellations, missed appointments, completed clinical evaluations, or administrative services may be nonrefundable. Contact the office for the policy that applies to your appointment type before booking.
Can I receive a receipt?
Yes. A receipt or visit statement may be requested. Whether an insurer reimburses a self-pay service depends on the patient’s benefits and plan rules; reimbursement is not guaranteed.

How quickly can I be seen?
Appointment availability varies. Same-day appointments may be offered when the schedule allows, but availability is not guaranteed. Emergency concerns should never wait for a telehealth appointment.
What do I need for a telehealth visit?
Please be ready in a private, well-lit location and have the following available:
- A smartphone, tablet, or computer with a camera and microphone
- A reliable internet connection
- A valid photo ID
- Your current medication list and allergy information
- Your preferred pharmacy name, address, and phone number
- Relevant home readings, such as temperature, blood pressure, heart rate, weight, or oxygen level, if available
- Your preferred payment method
Do I need to download an app?
Instructions depend on the secure telehealth platform used for your appointment. Your confirmation message will explain whether you can join through a browser or need an application. Test your camera, microphone, and connection before the visit.
What happens if the connection fails?
The practice may attempt to reconnect, contact you by phone, reschedule, or recommend in-person care elsewhere. The safest option depends on your symptoms and whether enough information can be obtained remotely.
Is my information protected?
Prime Vitality uses privacy-conscious clinical workflows and technology intended for healthcare communication and recordkeeping. Patients should join from a private location, avoid public Wi-Fi when possible, and protect appointment links and portal credentials.
Can I join a visit while driving?
No. For safety and privacy, patients should be parked in a secure location before joining. The provider may pause or reschedule a visit if the patient is driving or cannot participate safely.
Where must I be located during the visit?
You must tell the provider your physical location at the start of the visit. Care can be provided only where the provider is legally authorized to practice and where the requested service is appropriate.
Can you provide a work or school note?
A brief work or school excuse note may be provided when medically appropriate and supported by the evaluation. Notes are not guaranteed, are generally not backdated, and reflect only the condition and time period clinically supported by the visit.
Do you complete FMLA, disability, accommodation, or return-to-work forms?
These forms often require an established treatment relationship, detailed functional assessment, records review, ongoing management, or an in-person examination. Prime Vitality reviews requests individually and may decline or refer the patient to an appropriate primary-care clinician, specialist, occupational-health service, or employer-designated clinic.
How will I receive follow-up instructions?
After the visit, the provider will explain the plan, medication instructions when applicable, warning signs, and recommended follow-up. Instructions may also be delivered through the available secure clinical system, email process, or other method used by the practice.
How do I access my medical records?
Contact Prime Vitality at 702.521.0152 or info@primevitalitymedicalwellness.com to request records or ask how to access the available patient portal. Identity verification and a signed authorization may be required. Processing time and permitted fees follow applicable law.
How do I ask a non-urgent question after my visit?
Use the contact method provided in your visit instructions, call the office, or send an email. Email should not be used for urgent concerns or detailed sensitive medical information unless the practice has instructed you to use a secure method.
What if my symptoms worsen?
Follow the return precautions given during the visit. Seek urgent or emergency evaluation when symptoms worsen rapidly, new warning signs appear, or you are unable to safely wait for a response from the practice.
How are laboratory or imaging results handled?
The provider will explain the expected follow-up process when testing is ordered. Patients should complete testing promptly and keep their contact information current. A normal or abnormal result may still require a scheduled follow-up visit for interpretation and treatment planning.
Can Prime Vitality communicate with another clinician?
With your authorization and when permitted by law, relevant records may be shared with another clinician or facility to support continuity of care. Emergency disclosures may be handled under applicable privacy rules.
Telehealth is not an emergency service. If you believe you are experiencing a medical emergency, call 911 or go to the nearest emergency department. For urgent symptoms, do not rely on email, voicemail, or portal messaging.

We use reasonable administrative, technical, and physical safeguards designed to protect the information we collect. No method of transmission over the internet or electronic storage is 100% secure, and we cannot guarantee absolute security.
7. Children’s Privacy
Our website is not directed to children under 13, and we do not knowingly collect personal information from children through the website outside of the context of a parent or guardian scheduling care on a minor’s behalf.
8. Your Choices
Nevada residents have certain rights regarding the sale of covered information under Nevada’s online privacy law (NRS 603A). We do not sell personal information collected through this website. - Telehealth involves the use of electronic communication to enable your provider to evaluate, diagnose, and treat you remotely, and has both benefits (convenience, access) and limitations (no in-person physical exam, potential technology interruptions) compared to an in-person visit.
- Your provider may determine that your condition requires an in-person evaluation, and if so, will refer you accordingly.
- All existing confidentiality protections that apply to your medical information under HIPAA apply equally to information shared during a telehealth visit.

Restricted Controlled Substance Prescribing Policy.
What to know about controlled substance prescribing before you schedule a visit.

Prime Vitality Medical & Wellness PLLC maintains a strict restricted controlled-substance prescribing policy.
The practice does not prescribe, refill, renew, replace, or manage any DEA Schedule I or Schedule II controlled substance, or any opioid medication classified under Schedule III, whether the patient is evaluated through telehealth or during an in-person visit.
This Policy Includes, But Is Not Limited To
- DEA Schedule I controlled substances
- DEA Schedule II controlled substances
- Schedule II opioids and narcotic pain medications
- Schedule II stimulants, including controlled ADHD medications
- All opioid medications classified under DEA Schedule III
- Opioid-use-disorder medications classified as Schedule III controlled substances
If You Need a Restricted Medication
Patients who need a medication prohibited by this policy should contact their established primary care provider, psychiatrist, pain management specialist, addiction medicine specialist, or another appropriately authorized clinician.
No Obligation to Prescribe
Scheduling or completing an appointment with Prime Vitality Medical & Wellness PLLC does not create an obligation to prescribe any medication. Medications may be prescribed only when medically necessary, legally permitted, and clinically appropriate.
Medical Emergencies
Patients experiencing an overdose, severe withdrawal symptoms, seizures, difficulty breathing, chest pain, severe confusion, suicidal thoughts, loss of consciousness, or another emergency must call 911 or go to the nearest emergency department.

By scheduling or participating in an appointment, the patient acknowledges and agrees to comply with this policy.

These Terms of Service (“Terms”) govern your use of this website and the scheduling and telehealth services (and any in-person services, when available) offered by Prime Vitality Medical & Wellness PLLC (“Prime Vitality,” “we,” “us,” or “our”). By using this website, scheduling an appointment, or purchasing a membership or visit, you agree to these Terms. If you do not agree, please do not use the website or our services.
2. Not a Substitute for Emergency Care
This website and our telehealth services are not intended for medical emergencies. If you are experiencing a medical emergency, call 911 or go to the nearest emergency room immediately. Use of this website does not create a provider-patient relationship; that relationship begins only once an appointment is scheduled and a qualifying visit takes place.
3. Eligibility & Accurate Information
- You must be physically located in a state where the provider is licensed at the time of any telehealth visit, and you are responsible for confirming this.
- You agree to provide accurate, current information when scheduling, completing intake forms, or communicating with the practice.
- Care for a minor must be arranged and consented to by a parent or legal guardian.
4. Scheduling & Use of Online Tools
Appointments are booked through our online scheduling tool (Google Calendar) and telehealth visits are conducted through Google Meet, as described in our Privacy Policy and Notice of Telehealth Consent. You agree to use these tools only for legitimate scheduling purposes and not to submit false, abusive, or fraudulent booking requests.
5. Payment, Cancellation & Refund Policy
This section describes how billing, cancellations, no-shows, and refunds work for self-pay visits and memberships. Specific fee amounts, where applicable, are disclosed on our Payment & Pricing page or communicated to you before any fee is charged.
- Payment due at time of service. Self-pay charges are due at the time of service unless other arrangements have been made in advance.
- 24-hour cancellation window. Appointments cancelled or rescheduled with at least 24 hours’ notice are not charged a cancellation fee.
- Late cancellations & no-shows. Appointments cancelled with less than 24 hours’ notice, or missed without notice, may be charged a flat cancellation/no-show fee. You will always be told the applicable amount before it is charged.
- Completed visits are non-refundable. Once a clinical evaluation, telehealth visit, or administrative service has been completed, the fee for that service is non-refundable, regardless of the treatment outcome or whether a prescription, referral, or additional treatment was or was not provided.
- Membership fees. Wellness Membership fees are billed on the cycle disclosed at the time of purchase. Membership fees already billed for a current billing period are non-refundable once the period has begun; you may cancel to stop future billing as described in our membership terms provided at enrollment.
- Products & medications. Fees for prescriptions, laboratory testing, imaging, medications (including hormone or GLP-1/weight-management medications), and other third-party services are separate from visit fees, are billed by the applicable pharmacy, lab, or vendor, and are subject to that vendor’s own refund policies — not this Policy.
- How to cancel or reschedule. Call 702.521.0152 or email info@primevitalitymedicalwellness.com as early as possible before your scheduled time.
- Billing disputes. If you believe a charge was made in error, contact us using the information above and we will review the charge and respond promptly.
- Card & bank details. Payments are processed by our secure third-party payment processor; card and bank details are never stored on this website.

6. Restricted Controlled Substance Policy
The practice does not prescribe certain controlled substances, as described in our Restricted Controlled Substance Prescribing Policy. Scheduling or completing an appointment does not create an obligation for the practice to prescribe any medication.
